An independent investigation has been launched following the death of a 32-year-old inmate at HMP Chelmsford.

Bradley Gower died at the Chelmsford prison on 8 August 2026.

The Prisons and Probation Ombudsman has now confirmed that it is independently investigating the circumstances surrounding his death.

The PPO formally published details of the investigation on 3 September and currently lists its status as "in progress".

Inquest opened into death

An inquest into Mr Gower's death has also been opened by Essex Coroner's Service.

Official records show that the opening took place at Seax House on 26 August.

The coroner's record confirms that Bradley Michael Gower was 32 when he died in Chelmsford on 8 August.

No conclusion as to the cause or circumstances of his death has yet been published.

Independent investigation under way

The Prisons and Probation Ombudsman investigates deaths of prisoners and certain other people held in custody independently of the Prison Service.

Its investigation into Mr Gower's death remains ongoing.

The PPO generally publishes a final fatal-incident report only after its investigation has been completed, the findings have been shared with the deceased person's family and the coroner's inquest has taken place.

The circumstances and cause of Mr Gower's death should therefore not be assumed while those processes remain under way.

Further information is expected to emerge through the independent investigation and the coronial process.
